Please rememeber that for routine physical exams they usually take 45-90 minutes depending on how many tests need to be done.  These test may inclued EKGs, X-rays, blood tests, guiac (stool test for blood), urinalysis, Pap smear, or prostate exam.  In addition to the tests is a physical examination. 
This is an annual preventative exam and will be billed accordingly.  Please check with your insurance company, prior to your visit to verify your coverage, and make sure you have coverage for a preventitive exam and services.
To prepare for this physical, please fill out the above physical form (Health Questionnaire).  Please come to your appointment fasting, that is nothing to eat or drink after midnight with the exception fo water.  Bring the health questionnaire form completed along all medications you are currently taking.
